Specialty care doesn't stop between visits. ISHI Health is an AI-powered specialty care platform that enables multidisciplinary clinical teams to deliver continuous, personalized, guideline-directed care for heart failure and cardiometabolic patients between visits.

ISHI extends specialty care beyond the clinic through multidisciplinary clinicians, structured care pathways, and AI-enabled workflows. Nurse navigators, pharmacists, and nurse practitioners handle outreach, education, and medication support, while the practice maintains visibility and control. The ISHI app connects a practice's NextGen Enterprise EHR to the ISHI care team, so referring a patient and receiving ISHI's work both happen inside the chart.

How it works

1. Refer from the chart. A clinician places an ISHI referral order on the patient's encounter, with the referring diagnoses. No fax, portal, or phone call is needed.

2. Automatic onboarding. ISHI detects the new referral order within minutes and creates the patient in the ISHI platform with demographics, contact details, and the referring diagnoses from the order.

3. Enrollment and care. The ISHI care team contacts the patient and enrolls them in a personalized care pathway with continuous monitoring, engagement, and medication support.

4. Results back in the chart. Signed monthly program reports are filed into the patient's chart as documents under a dedicated ISHI Reports document type, so the practice sees ISHI's work without leaving NextGen.
