What is clinical documentation?

“I’m not doing notes anymore.”
When Dr. Keval Parikh started as a foot and ankle surgeon, he found himself staying at the office until 7:00, 8:00, or even 9:00 in the evening to complete all his clinical notes. Eventually, he decided to start going home after he saw patients. However, he was still spending his evenings completing notes after dinner alongside his wife, who is a physical therapist.
“I always saw myself at that point kind of catching up. I spoke with my partner like, ‘there has to be a better way.’” After researching different options, Dr. Parikh signed on for a trial of NextGen® Office Clinical AI to see if it could help reduce his after-hours charting and allow him to stop playing catch-up.
“After about two weeks [of the trial], I decided…I’m just going to get my own license because I kind of like it. From there, I think I've changed the way that I've been practicing. As a practicing gynecologist for 21 years, Dr. Bani Bajaj remembers when paper notes were full of illegible chicken scratch, but at least they were left at the office, and she went on with her day.
Since transitioning to the EHR in 2009, she slowly realized how much of her life clinical documentation began to take over. “I was literally drowning.”
On a typical day, she would wake up at 4:30 AM to review portal messages and labs. After her day in the office, Dr. Bajaj says she “would do charts at home throughout the evening. Never see my kids, not cook dinner, not do anything. Then go to bed at 9 –10 PM.”

Dr. D’Andra D. Bingham has been in private practice in gynecology and obstetrics for over 20 years.
